Change Your Details

You can update your personal details using any of the three options below:

  1. Online: Use Online Change of Details anytime to update your personal details.
  2. Phone: Contact the Information line on the telephone number listed below.
  3. Change of Personal Details Form: Download, complete and mail the Change of Personal Details Form to the Department of Justice.

You can use any of these methods to change or update your:

  • address
  • phone number/s
  • employer / volunteer organisation name and address
  • child-related work codes

Please Note: As a name change requires a new card you need to use the Replacement Card Form, to apply for a replacement card.
