Replacement Card Form
Complete this form if you require a replacement Working with Children Check Card, for example if your card is lost, stolen or damaged.You can either complete the form online and print it out to send or print it out and complete by hand.
A fee applies for the issuing of a replacement 'Employee' card. The fee is $18.20.
There is no fee to replace a 'Volunteer' card.
