Application for Revocation Form

This form is the 'Application For Revocation' of an infringement notice.

Form A should be completed to revoke a vehicular infringement notice if you:

• were not the driver of the vehicle, or
• are not the owner of the vehicle.

Form B, 'Application For Revocation, Special Circumstances and Other' should be completed if you believe special circumstances exist, that mean you should not have to pay or should be given an extension of time to pay the penalty.
