Evidence of Name Change

If the name on your application form is different from the name on your identification documents, for example if some documents are in your maiden name, you must also provide an evidence of change of name document when lodging your application.

Documents that can be accepted as evidence of a name change must state your current name, your previous name and your date of birth. These may include:

  • your marriage certificate
  • a change of name certificate issued by a state registry of births, deaths and marriages
  • divorce papers
  • a birth certificate issued in your new name
  • a deed poll.
