Replacement Card Form
Complete this form if you require a replacement Working with Children Check Card, for example if your card is lost, stolen or damaged.
A fee applies for the issuing of a replacement 'Employee' card. From 1 July 2009, the fee is $17.40.
There is no fee to replace a 'Volunteer' card.
