Proof of Age
The Victorian Proof of Age Card is a form of identification that can be used as evidence of age when seeking to enter licensed premises or purchase liquor (and many other purposes). The card is recognised throughout Australia.
Who can apply and how much does it cost?
Any person who has reached the age of 17 years and 11 months can apply, however you must wait until you are 18 to use it. The application fee is $10.
Where do I go to apply?
You can pick up a Proof of Age Card application form at Consumer Affairs Victoria (CAV) or any approved photo point agency (download a complete list of approved photo point agencies via the link below), including:
- participating Australia Post offices
- participating VicRoads offices
- selected pharmacies or chemists
- certain photography and other stores
You can also request a form to be posted to you by calling 1300 650 367.
How to apply
Complete the Proof of Age Card application form. Making a false statement is a serious offence.
Step 1:
Gather proof of identity. Two types of proof of identity are required, one from each category below (original documents are required for sighting, photocopies are unacceptable). If you have changed your name (either through marriage or otherwise) you must also provide documentary evidence of the change.
Category A
- Full Birth Certificate
- Passport
- Naturalisation Certificate
- Immigration Papers
- Citizen Papers
Category B
- Drivers Licence or Learners Permit
- Social Security Card
- Credit Card or Bank Passbook
- Medicare Card
- Shooters Licence
- Student Identification Card.
Step 2:
Take the completed application form and your identification documents to an 'Authorised Referee' and sign the form in front of them. An 'Authorised Referee' is a person who:
- can witness the signing of a statutory declaration (a list of qualified persons can be found inside the application form)
- has known the applicant for at least 12 months
- is on the electoral role.
- The Authorised Referee must then complete and sign the Authorised Referee declaration on the application form.
Step 3:
Go to a Photo Point agency (see the attached document Approved Photo Point Agencies) to get your photograph taken and lodge the application form. Take along your identification documents as the person from the agency is required to check your documents and also sign the application form where indicated.
